Community Outreach Worker-Adult Day Health Care Part Time- 20 hours per week

GuildCare, an innovative community-based Adult Day Health Care Program providing services to adults with health and vision needs is seeking an energetic and autonomous individual interested in part-time employment working 20 hours per week as an Outreach Worker for their Buffalo location.
This individual will be responsible for promoting the GuildCare program, seeking out Medicaid-eligible clients in need of Adult Day Health Care services through visits and presentations at community centers, subsidized housing areas, healthcare agencies, etc.
and obtaining appropriate referrals.
A BSW or BS in Human Services preferred, ability to provide own transportation for local travel and computer proficiency required.
Experience in outreach or marketing or case management preferred.
Must be familiar with local community services and demonstrate excellent customer service skills .
Healthcare experience preferred.
